Given the success of the chronic care model with La Clínica's diabetic patients, the high prevalence of asthma among pediatric patients, and staff interest in improving services for this population, the health center team submitted an application to become a member of the national BPHC Asthma Collaborative in 2002. Self-management improvements include devoting Clínical resources to individual patients and/or family one-on-one education regarding how to use peak flow meters and asthma medication so that patients are knowledgeable about and comfortable with methods to improve their health. Staff have adopted evidenced-based care guidelines, developed an asthma progress note form for providers to use, and assembled asthma resource tool kits. La Clínica's commitment to improve care for asthmatic patients is substantiated by staff participation in the development of the Regional Asthma Management Program (RAMP).
