La Clínica's innovative tracking of diabetes and asthma through a computerized patient registry allows chronic disease patients an integrated and comprehensive level of care that is rare among community health centers in the nation. The diabetes flow sheet is printed from the registry, which stores key Clínical indicators, triggers flags for follow up and action, and thus allows better-targeted management of care. The registry is accessible to any care provider and flow sheets are produced for use at each office visit and placed charts. A similar registry and flow sheets for asthma patients allows this type of targeted and coordinated care for our pediatric asthma patients. The registries contain information for each diabetic and asthmatic patient, including medical history, lab results and medications, and monthly reports for providers.
Helping patients to make lifestyle changes is an essential component of the Chronic Care Model. Our health education department offers a variety of services relating to nutrition (weight control and healthy weight loss); cardiovascular health; cholesterol reduction; smoking cessation; healthy Latin-American cooking techniques; and asthma control.
