ABOUT THE POSITION: Under general supervision, the individual will provide care management and HIV prevention education on the streets and in other non-traditional settings to those not reached by current health education efforts, primarily day laborers and also including men who have sex with men (MSM), the homeless, injection drug users, sex industry workers, runaway youth, undocumented immigrant and their partners.
MINIMUM QUALIFICATIONS:
  1. Requires High School diploma or equivalent supplemented by one or two years experience working with the above groups, as well as experience with day laborers issues, HIV issues, community health education, and/or community organizing.
  2. Ability to work with people of diverse ethnic, economic, and educational backgrounds, intravenous drug users, people of different sexual orientations, and people living with HIV/AIDS.
  3. Works independently as well as in a team; maintains good working relationships with staff and community members.
  4. Familiarity with Fruitvale/East Oakland area preferred but not required.
  5. Must have a flexible schedule and be able to work evenings and weekends.
  6. Ability to keep clear, accurate, and legible notes and statistics.
  7. Good writing skills and basic computer skills required.
  8. HIV Test Counselor certification recommended.
  9. Bilingual English/Spanish required.
DUTIES AND RESPONSIBILITIES:
  1. Develops strategies for outreach to promote and publicize the Jornaleros Project and HIV Services on the street and in non-traditional settings, to day laborers.
  2. Provides education and information about La Clinica medical services, community services and HIV transmission and prevention practices to day laborers.
  3. Performs “warm-hand off” from La Clinica registration to La Clinica services and community services.
  4. Collaborates effectively with staff at Street Level Health Center and other partner agencies.
  5. Provides referral and follow up to referrals of day laborers to La Clinica medical services and community services.
  6. Advocates for clients to access treatment programs, mental health services, housing, health and other social services.
  7. Documents outreach activities through weekly field notes, collects information, and maintains accurate and complete records to be reviewed by supervisor.
  8. Responsible for client contact, tracking and follow up.
  9. Develop, test and conduct surveys, interviews and focus groups as par of the program evaluation.
  10. Network with other agencies providing services to the target population.
  11. Participate in health fairs, community events, and other departmental activities.
  12. Other duties as assigned by supervisor.
